The EyeMed Vision Care Plan provides benefits for routine eye exams and necessary eyewear every 12 months.

Eye Examination	$20 copay	Plan pays up to $32
Eyeglass Lenses Single Vision, Lined Bifocal, Lined Trifocal	$20 copay	Separate $100 allowance toward eyeglass lenses or contact lenses, but not both
Frames	$175 allowance, then 20% discount on remaining balance	$100 allowance toward frames
Contact Lenses (in lieu of eyeglasses)	$175 allowance	Separate $100 allowance toward eyeglass lenses or contact lenses, but not both
